Fluoride-Free Dentistry
I choose hydroxyapatite as my preferred approach to remineralization and prevention.
My approach to dentistry is fluoride free. I have concerns about unnecessary systemic fluoride exposure, particularly in light of evolving research around neurodevelopment.
At the same time, I do not believe abandoning fluoride should mean abandoning prevention. We now have a biomimetic alternative that makes biological sense to me: hydroxyapatite, a calcium-phosphate mineral closely related to the mineral already present in teeth.
Why I Choose Fluoride-Free Care
When an effective alternative exists, I prefer a precautionary approach.
Fluoride can have neurotoxic effects at sufficiently high exposure.
That does not mean every level of fluoride exposure has been proven to cause neurological harm. Dose matters. Route of exposure matters. Timing matters.
The 2024 National Toxicology Program review found a consistent association between higher fluoride exposure and lower IQ in children. The evidence was strongest at exposures around or above 1.5 mg/L in drinking water.
The same federal review did not determine whether 0.7 mg/L, the level recommended for U.S. community water fluoridation, negatively affects children’s IQ. There were not enough data to answer that question.
Fluoride + Neurodevelopment
What the National Toxicology Program actually concluded.
This is where precision matters.
I do not need to exaggerate the research to explain why I choose a fluoride-free approach.
NTP concluded that higher estimated fluoride exposure is consistently associated with lower IQ in children.
Fluoride-Free Water Advocacy
My fluoride-free philosophy extends beyond the dental chair.
I advocate for fluoride-free public water because I believe exposure should be an individual, informed choice.
People can choose whether to use a fluoride-containing toothpaste, varnish or other dental product. Community water fluoridation is different because exposure occurs through a shared public water supply.
Given the evolving research around fluoride exposure and neurodevelopment, I believe the precautionary principle deserves a place in that public-health conversation.
My position is not that prevention should disappear if fluoride is removed from water. It is the opposite. We should improve prevention through education, nutrition, dental access, oral hygiene, individualized risk assessment and effective alternatives such as hydroxyapatite.
The Fluoride-Free Alternative
Hydroxyapatite speaks the tooth’s mineral language.
Hydroxyapatite is a calcium-phosphate mineral closely related to the mineral phase that naturally gives enamel and dentin their structure.
In oral-care products, hydroxyapatite particles can interact with enamel surfaces, deposit within microscopic irregularities and support remineralization of early mineral loss.
This is one reason hydroxyapatite fits naturally into my biomimetic philosophy. We can think about restoring mineral to the tooth itself.
Hydroxyapatite Research
The evidence base is no longer just laboratory theory.
Hydroxyapatite has now been tested in human clinical trials and systematic reviews.
The evidence base is newer and smaller than the decades of literature behind fluoride. That distinction is worth saying clearly.
In adults, a fluoride-free hydroxyapatite toothpaste met the study’s predefined noninferiority criterion compared with a 1450-ppm fluoride toothpaste for the primary caries outcome over 18 months.
Read the clinical trial on PubMed →A systematic review of hydroxyapatite-based, fluoride-free toothpastes concluded that HAp could be an effective alternative for preventing caries progression and supporting enamel remineralization.
Read the review on PubMed →A 2024 systematic review found that evidence supporting fluoride-free hydroxyapatite oral-care products for caries prevention has expanded across randomized and in-situ clinical studies.
Read the review on PubMed →Research continues to evolve. Product formulation, hydroxyapatite concentration, brushing habits, diet, saliva and baseline caries risk all matter clinically.
At Your Cleaning
We use hydroxyapatite varnish instead of fluoride varnish.
My fluoride-free philosophy does not stop at the toothpaste I recommend at home.
When a varnish application is appropriate during a hygiene visit, our office can use a professional-strength hydroxyapatite remineralizing varnish rather than fluoride varnish.
The goal is targeted mineral support for enamel and sensitive areas while remaining consistent with the fluoride-free philosophy of the practice.
What I Recommend at Home
Build a fluoride-free routine around hydroxyapatite.
You do not need a cabinet full of oral-care products. Start with a strong daily foundation and add targeted support when it serves a purpose.

Why I choose a fluoride-free approach.
My position is not simply “take fluoride away.”
Prevention still matters. Remineralization still matters. Caries prevention still matters.
What has changed is that we now have a clinically studied alternative that allows me to practice consistently with my concerns about fluoride exposure without giving up on mineral support for the teeth.
That alternative is hydroxyapatite.
Prevention Is Bigger Than Fluoride
Fluoride-free dentistry should be more intentional, not less.
Toothpaste is one variable in an entire oral ecosystem.
Fluoride-Free Hygiene Visits
Your cleaning can stay consistent with your fluoride-free philosophy.
Preventive dentistry should be personalized to the patient in front of us.
At hygiene appointments, we evaluate much more than whether plaque was removed. We consider gum health, caries risk, saliva, sensitivity, home care, nutrition and the oral environment.

Questions
Fluoride-Free Dentistry FAQs
Why don’t you use fluoride?
My practice takes a precautionary, fluoride-free approach. Research reviewed by the National Toxicology Program has associated higher fluoride exposure with lower IQ in children. Evidence is strongest at higher exposure levels, while available data do not establish whether 0.7 mg/L community-water fluoridation affects IQ. Because an increasingly studied fluoride-free alternative exists, I prefer hydroxyapatite for remineralization and preventive oral care.
Is fluoride a neurotoxin?
Fluoride has neurotoxic potential at sufficiently high exposure. The 2024 National Toxicology Program review concluded with moderate confidence that higher estimated fluoride exposures are associated with lower IQ in children. That does not establish that every level of fluoride exposure causes neurological harm.
Do you support fluoride-free public water?
Yes. I advocate for fluoride-free water and believe people should be able to make an informed individual choice about whether to use fluoride-containing dental products. I have also spoken publicly before the North Miami Beach City Council in support of removing fluoride from the public water supply.
Watch my City Council speech →What is hydroxyapatite?
Hydroxyapatite is a calcium-phosphate mineral closely related to the mineral phase naturally found in enamel, dentin and bone. In oral-care products, it is used to support remineralization and help manage sensitivity.
Does hydroxyapatite help prevent cavities?
Clinical evidence is growing. Randomized clinical trials and systematic reviews support hydroxyapatite toothpaste as a fluoride-free caries-prevention option. The evidence base is newer and smaller than the long-established fluoride literature.
Is hydroxyapatite toothpaste as effective as fluoride toothpaste?
Several clinical studies have reported comparable outcomes. An 18-month randomized adult trial found that a hydroxyapatite toothpaste met the study’s predefined noninferiority criterion compared with a 1450-ppm fluoride toothpaste. That does not mean every hydroxyapatite formulation is automatically equivalent to every fluoride formulation.
Do you use fluoride varnish at cleanings?
My practice is fluoride free. When a remineralizing varnish is appropriate, we can use a professional-strength hydroxyapatite varnish instead of fluoride varnish.
What fluoride-free toothpaste do you recommend?
I generally recommend hydroxyapatite-based toothpaste. My current fluoride-free recommendations include FYGG hydroxyapatite products, which you can explore through the Mindful Doctor shop.
Explore my fluoride-free recommendations →Does going fluoride free mean I can ignore diet?
No. Tooth decay is influenced by plaque biofilm, sugar and carbohydrate frequency, saliva, hygiene, microbial ecology, existing dental work and individual risk factors. Fluoride-free prevention should be comprehensive, not product-dependent.
How do I schedule fluoride-free dental care in South Florida?
Request an appointment through Mindful Doctor. We can evaluate your oral health, caries risk, sensitivity, current products and preventive needs and build a fluoride-free strategy appropriate for you.

Fluoride free does not mean prevention free.
It means building a preventive strategy around hydroxyapatite, oral hygiene, nutrition, saliva, microbial balance and understanding your individual risk.
